The situation

What has happened

The draft October crew roster is due for publication on 28 September. In September a cabin crew member reported fatigue on the fourth day of a duty block after a roster change reduced rest to the minimum, logged as SR-26-0143, and two pilots and one senior cabin crew member have checks expiring in October. The Head of Flying Operations wants a written review of the draft roster against the Fatigue Risk Management Policy limits, an analysis of where September roster changes reduced rest, and a check that nobody is rostered beyond a check expiry in the Crew Training and Qualification Records. The review must recommend changes and explain how crews will be briefed so that the published roster is both compliant and accepted.

What to look for

Evidence guide

Provide a draft roster with at least three lines that breach or sit at limits and one that crosses a check expiry. Look for every issue identified, the policy limits correctly applied, practical recommendations that keep the operation running, and a crew briefing that is respectful and consistent with just culture. Residual risk should be identified honestly.
