Safety Occurrence and Hazard Report Form

Purpose. Any worker completes this form to report an occurrence, hazard, fatigue concern, security incident or dangerous goods event so that it is recorded in the Safety Reporting System and acted on.
1.When to use this form
Use this form for anything that affected or could have affected the safety or security of an aircraft, a person or an operation. This includes ground damage, near collisions on the ramp, loading errors, unstable approaches, cabin injuries, disruptive passengers, airside access breaches, undeclared dangerous goods and fatigue. If in doubt, report it.
2.Confidential reports
Tick the confidential box if you do not want your name shared beyond the safety and compliance manager. Confidential reports still receive feedback and are still investigated.
3.Completing the form
Describe what happened in the order it happened, using plain language. Include the flight number, aircraft registration or bay, the equipment involved and the names of witnesses. Give your honest view of what contributed and what would prevent it.
4.What happens next
The safety and compliance manager classifies the report within two business days and either assigns it to the department or appoints an investigator. You will be told the outcome and any actions taken.
5.Protection
Reports are used only for safety improvement under the just culture provisions of the Safety Policy.
